Where can I pick up a copy of a police report?
Copies of police reports can be obtained from the Records Unit located on the 1st floor of the Thomas R. Windham Building (Police Headquarters), 350 West Belknap in downtown Fort Worth, Monday through Friday from 7:00 a.m. to 6:00 p.m. Offices are closed on weekends and major holidays.
What is the cost for a police report?
Accident reports are available at a cost of $6.00 per report. Incident and Offense reports are available for $1.50. These reports can be certified for an additional fee of $2.00 for an Accident report and $5.00 for an Incident/Offense report. The cost of a letter certifying that no accident report exists is $6.00. The Records Unit accepts cash, no bills larger than $50.00, money orders, credit cards, and cashier checks made payable to the City of Fort Worth. Personal or business checks are not accepted
Are police reports available over the Internet?
Police reports are not available on the Internet at this time. The Operations Support Bureau is researching software applications that will allow us to offer this service. Notice will be posted on the website when this service becomes available.
Are police reports available by mail?
Police reports may be sent to you by mail or email, or can be picked up in person as stated earlier. To receive reports by mail, you must send in your name, date, report number, location of the accident, and the fee for the cost of the report. To receive reports by email, you must send in the previous information mentioned plus your email address. No more than ten (10) reports per request may be emailed to you, and no more than ten (10) pages per report may be emailed to you. If your request exceeds the email limits, your reports may be mailed to you instead. For easy ordering, you may print out and complete the Mail-In Customer Order Form located on this website and provide all information requested. You may pay by credit card, cashier’s check or money order if ordering by mail. Please include your credit card number on the form. Do not send cash through the mail. You may enclose additional pages listing report numbers, as needed. Please remember to sign the form and include a self-addressed stamped envelope(s). All Fed-Ex charges for mailing reports must be paid for by the requestor. No more than three (3) pages, including the order form can be faxed in to request police reports.

When should I use a Public Information Request Form?
Public Information Request forms are required when sexual assaults, senior citizens age 65 or above located in nursing homes, or juveniles are listed as suspects, arrested persons or victims. They must also be used when any incidents occur at mental health care facilities. You may print out and complete the Public Information Request form located on this website and mail it to the address indicated on the form. The costs associated with this form are also posted, however you will be notified of the total costs due after your form has been reviewed and approved by records management personnel.

How do I obtain a form to report an accident for insurance purposes?
The CR-2 Driver’s Crash Report (Blue Form) is available from any police officer or at any police sub-station. You may also download the form from the Texas Department of Transportation website. The website is www.txdot.gov and the form is listed under “Crash Reports and Records.”
What information can I receive regarding my arrest information?
Once an Identification Technician has verified your identity with a current government- issued photo ID (driver’s license, military ID, etc.), your arrest information is available to you at the cost of $5.00. Requests for arrest information regarding Mansfield detainees, must be submitted in writing to Mansfield Law Enforcement Center (MLEC). They can be contacted at 817-477-1343.
Can I obtain driver’s license and criminal history information?
Driver’s license, and state and national criminal history information (itemized dates of arrests, charges and dispositions) is not available from the Fort Worth Police Department because of our teletype agreement with the Texas Department of Public Safety. The Identifications Unit can provide you with your criminal history information as it relates only to the City of Fort Worth at a cost of $5.00. A current government-issued photo ID (drivers’ license, military ID, etc.) is required, unless you have an arrest history with fingerprints on file. The Department of Public Safety in Austin can provide this information, however you will need to contact them at 512-424-2000.
Does the City of Fort Worth offer fingerprinting services?
Fingerprinting services are available in the Identifications Unit located on the 1st floor of the Police Headquarters, 350 West Belknap, Monday through Friday from 7:00 a.m. to 6:00 p.m., at a cost of $10.00 per card. Citizens are required to bring the necessary card(s) and a current government-issued photo ID (driver’s license, military ID, etc.). For concealed handgun fingerprints, passport size photos must be presented. State Rejection letters and Fort Worth receipts are required for free reprinting of fingerprints.
How do I obtain a Clearance Letter?
The Police Identifications Unit provides Clearance letters indicating all record of arrests with FWPD at a cost of $5.00. You should bring a current government-issued photo ID (driver’s license, military ID, etc.) with you. If you live outside of the area, you will need to mail a copy of a current government-issued photo ID including your full name, race, gender, date of birth, and your social security number for exclusionary purposes. A cashier’s check or money order should be made payable to the City of Fort Worth. There is an additional $5.00 charge for a certified copy. Please include a self-addressed stamped envelope. Personal or business checks are not accepted.
If I live out of the area, can I still get a copy of my criminal history information?
You cannot receive a copy of your criminal history information by mail. The Identification Unit can only release this information in person.
